What is FMEA?


Failure Mode and Effects Analysis (FMEA) is a comprehensive engineering technique that manufacturers can use to improve the quality, reliability and safety of their products. The methodology provides a systematic approach for examining all the ways in which a device can fail to function as required. In particular, FMEA identifies, defines and eliminates known and potential failures, problems and errors from products, designs, systems and services before they reach the customer.

When should you use FMEA?


FMEA is designed to help you maximize the life of your fixed assets. It can be used at all stages of the lifecycle of your product. FMEA Methodology. The FMEA process involves the following steps:

1. Document the components and items that make up the device being analyzed.

2. Define the function of each item.

3. Identify all potential failure modes.

4. Determine the causes of each failure mode.

5. Assign a rating for severity, occurrence and detectability for each failure.

6. Identify tasks that may be performed by operations maintenance personnel.

7. Determine the most appropriate corrective/preventive actions based on the Risk Priority Number and/or severity.

8. Carry out the recommended actions.
