Bow Tie Analysis is a graphic alternative to traditional Hazard and Operability (HAZOP) and “What-If/Checklist” study methods that helps visualize the greatest risk management gaps early on in a project.  A qualitative PHA, it is less rigorous than a Quantitative Risk Assessment or Fault Tree Analysis and often used to better understand events and issues within a business and the controls and consequences of those events.  Bow Tie Analysis provides a visual representation of the causes of unintended events, likely outcomes, and the measures in place to mitigate or control the hazards. Existing safeguards are identified and evaluated on the right side with the post-event consequences on the left, serving as “wings” of the Bow Tie with the unintended event depicted in the center.  Due to its graphic nature, Bow Tie Analysis is easily understood by all levels of operations and management.
